This is the second theatrical film based on the TV drama "TOKYO MER: Mobile Emergency Room," which depicts the activities of a life-saving medical team (Mobile Emergency Room = MER) that rushes to accident and disaster sites in large vehicles equipped with operating rooms.

As TOKYO MER's achievements are highly praised, new MERs are being established in major cities across the country. Meanwhile, in Okinawa and Kagoshima, activities to attract MERs that can respond to remote island areas are becoming active. As instructors, Chief Doctor Kitami and Nurse Natsume from TOKYO MER are dispatched, and the trial operation of "Nankai MER," which responds to accidents and disasters on remote islands with a medium-sized vehicle equipped with an operating room, begins. However, after six months, there are no emergency calls, and it is on the verge of being abolished. One day, a volcano erupts on Suwanose Island in Kagoshima Prefecture, and the decision is finally made to dispatch to a large-scale medical incident. The situation on the island is far beyond imagination, and rescue by helicopter is impossible due to the volcanic smoke, and the arrival of the Maritime Self-Defense Force and the Coast Guard will be in several tens of minutes. Amidst flying rocks and approaching lava, Nankai MER takes on a high-difficulty mission to save the lives of 79 people stranded on the island.

In addition to the familiar cast, including Ryohei Suzuki as the protagonist Kitami, Yosuke Eguchi, Mahiro Takasugi, Meru Nukumi, and Emma Miyazawa appear as members of Nankai MER, and Tetsuji Tamayama newly appears as a local fisherman who works to save the islanders.
